The Montgomery County Board of Developmental Disabilities Services (MCBDDS) is conducting its annual Consumer Satisfaction Survey. The Board is asking individuals who receive services from the Board, along with their parents and guardians, to complete the survey in hard copy or electronic form by Friday, August 13 at 11:59 p.m.
The survey poses general questions and questions about specific MCBDDS departments and functions. If you do not receive services from a particular department, please skip that section and move to the next. Keep in mind that the questions pertain only to the previous 12 month period.
Please complete either the paper copy of the survey that was mailed to you OR the online version, but NOT both.
