MCBDDS will hold a Booster Shot clinic Friday, November 19 for people served and their caregivers who received the Pfizer, Moderna, and Johnson & Johnson vaccines more than six months ago and meet specific eligibility requirements.
The clinic will be held at Mt. Enon Missionary Baptist Church, 1501 W. Third Street, Dayton, 45402. Appointments will begin at 10 a.m. To qualify, a person must live or work in a congregate situation and meet the age and medical requirements in the following document: https://bit.ly/3lhL8hy
Those interested in receiving a booster may schedule an appointment by emailing help@mcbdds.org or by calling 937-457-2808, and must indicate which vaccine they wish to receive. The day of the clinic, they must bring their vaccine card, insurance, Medicare or Medicaid cards, and a completed Consent Form, which can be found here: https://bit.ly/2YnhXjU
