Getting NPI Number

Medicaid’s Provider Network Management (PNM) system requires all providers to have a National Provider Identifier (NPI) number. This includes Independent Providers, agencies, day services, and county boards.

The NPI is a unique, 10-digit identification number assigned to covered health care providers.

To obtain an NPI number, please consult the following documents, which can be used by both independent and agency providers:

  1. Creating a Username for NPPES
  2. Applying for an NPI Number
  3. Taxonomy guide for DODD waiver providers
  4. National Provider Identifier Application/Update Form
  5. Accessing the Medicaid Provider Number

For additional information, please review the NPI Tidbits document. Should you have any questions about this process, please contact Marci Slifko at mslifko@mcbdds.org or 937-457-2816.
